Athletic and Sports Physicals

Full physical examination will be performed in guidance with the IHSA guidelines.  Link below available to access physical forms.  Forms will be filled out at time of service. 

Available to do full team sport physicals for club, travel or school sports.  If performing sport physicals, Bee Well Healthcare will donate money back to the team or organization.

Link below provided for access to DOT physical forms.  I have been a licensed/certified DOT provider since 2014.  Full physical will be done plus urinalysis.  All paperwork will be filled out at time of visit.
